Important: SSA application policy change. Since August 2023, SSA no longer accepts faxed disability applications as valid. You must apply online or with a signed original paper form. However, SSA still accepts faxed supporting documents, appeal forms, medical evidence, and authorization forms. A faxed application can still establish a protective filing date.

Attach the SSA barcode (if applicable)
