Fax IRS Form 15397: extend time to furnish statements

Request up to 30 more days to give certain tax statements to recipients, not to file them with the IRS.

IRS sources checked October 13, 2026. Verify current instructions before sending.

Before you send

Form 15397 extends time to give statements to recipients only. It does not extend the IRS information-return filing deadline.

What Form 15397 does

Form 15397 requests a one-time extension of up to 30 days to furnish certain information-return statements to recipients. It does not extend the deadline to file information returns with the IRS; Form 8809 serves that different purpose and cannot be faxed.

Check the form before faxing

  • Check that the statements you need to furnish are among the form types listed on the current Form 15397 and explain why more time is needed.
  • Complete and sign the form. File it no later than the due date for the recipient statements; if several statement types have different due dates, the earliest one governs a combined request.
  • Choose the IRS online submission or fax. Do not send the same extension request more than once, because the form warns duplicates can delay or prevent processing.

The May 2026 Form 15397 lists the domestic and international fax numbers below for the IRS Technical Services Operation, Attn: Extension of Time Coordinator. It also offers an online form.

Send Form 15397

OneFaxNow sends outbound faxes only. It does not prepare the IRS form or confirm IRS acceptance.

After the fax is delivered

Keep the signed request and delivery record. The IRS says it does not issue an approval letter for a complete, approved request; it sends a letter only if the request is incomplete or denied. Fax delivery alone does not grant the extension.

Questions about Form 15397

Does Form 15397 extend the deadline to file Forms 1099 with the IRS?

No. It concerns recipient statements. IRS filing extensions are a separate process; the IRS says Form 8809 extension requests sent by fax are not accepted.

Will the IRS mail an approval letter?

The current form says no approval letter is issued for an approved request. The IRS sends a letter if the request is incomplete or denied.

OneFaxNow is not affiliated with the IRS and does not provide tax or legal advice. A fax transmission record does not establish that the IRS processed or accepted a form.
