What Form 4506-T does
Form 4506-T requests a transcript of a tax return or other tax account information. It requests a transcript, not a photocopy of the full filed return. Check whether the IRS online transcript service can provide what you need before faxing a request.
Check the form before faxing
- Choose the exact transcript type and tax years on the form. The routing chart separates individual Form 1040-series, W-2 and 1099 transcripts from other transcripts.
- Use the state where you lived, or your business was located, when the return was filed. Your current address may lead to a different IRS team.
- Complete the signature section and its authorization checkbox. For a joint return, at least one spouse must sign. The IRS must receive Form 4506-T within 120 days of signing.
Where to fax Form 4506-T
The live IRS where-to-file page has two separate charts. First select the type of transcript, then locate the state associated with the return you filed. If one request covers transcripts that map to different offices, the IRS says to send it to the office listed for the address on your most recent return.
The IRS says this live page can be newer than the printed form instructions. Use the live chart immediately before you send; this guide does not prefill a fax number because the correct one depends on your request.
