Fax IRS Form 14039: identity theft affidavit

Report qualifying tax-related identity theft. A notice-specific fax route takes priority over the general number.

IRS sources checked October 13, 2026. Verify current instructions before sending.

Before you send

If an IRS notice gives a different fax number, use that number. If the notice has no fax number, follow its mailing instructions. Do not fax an original tax return to the identity theft line.

What Form 14039 does

Form 14039 tells the IRS about certain tax-related identity theft affecting an individual or dependent. Most people do not need to file it: check the IRS identity theft guide first, especially if the IRS already sent you a letter or you used its identity verification service.

Check the form before faxing

  • Confirm that your situation matches a reason listed in Section B. Do not send another Form 14039 for an incident you already reported.
  • Complete Sections A through E, including the signature. Complete Section F and attach the required documents if you are filing for someone else.
  • Include a cover sheet marked “Confidential.” Choose one submission method: online, mail, or fax. The IRS says not to use all three.

Where to fax Form 14039

If you are responding to an IRS notice or letter, send Form 14039 to the fax number on that notice when one is provided. If the notice has no fax number, follow its mailing instructions instead.

For other eligible submissions, the February 2026 form lists the general identity theft fax number below. This line is not for an original tax return.

No notice-specific route: 855-807-5720

Use only when the form’s general fax instructions apply; a fax number on an IRS notice takes priority.

Send Form 14039

After the fax is delivered

Keep the signed affidavit and delivery confirmation. The IRS must review the report separately; a delivered fax does not mean it has marked or corrected your tax account.

Questions about Form 14039

Should I fax Form 14039 if the IRS sent me a notice?

Read that notice first. If it gives a fax number for the affidavit, use it. If it does not, the Form 14039 instructions say to follow the notice’s mailing directions.

Can I send my tax return to the Form 14039 fax number?

No. The IRS says to send an original return to its normal filing destination, not the identity theft affidavit fax line.

OneFaxNow is not affiliated with the IRS and does not provide tax or legal advice. A fax transmission record does not establish that the IRS processed or accepted a form.
