Fax IRS Form 14039-B: business identity theft

Report suspected business identity theft or an EIN you never requested. Notice instructions determine the route when you received one.

IRS sources checked October 13, 2026. Verify current instructions before sending.

Before you send

Do not use the general identity theft number when an IRS notice directs you elsewhere. The form calls for different supporting documents depending on the affected entity.

What Form 14039-B does

Form 14039-B reports suspected identity theft involving a business, estate, trust, or exempt organization. It also has a separate option for someone who did not request an EIN but receives IRS notices about an unknown business in their name.

Check the form before faxing

  • Choose the correct box in Section A. The form identifies different sections to complete for an unknown EIN and for an affected business or other entity.
  • Sign Section F. If reporting theft involving an entity, review Section E for the supporting identity and business documents required for that entity type.
  • If the IRS sent a notice, read its response instructions before entering any fax number.
