Fax IRS Form 15227: apply for an IP PIN

Apply for an Identity Protection PIN by fax if you meet the form’s eligibility rules and cannot use the faster online option.

IRS sources checked October 13, 2026. Verify current instructions before sending.

Before you send

The IRS requires a cover sheet marked “Confidential” for faxed Form 15227. Faxing does not give you an immediate IP PIN; the IRS first contacts you to verify your identity.

What Form 15227 does

Form 15227 lets eligible taxpayers request an Identity Protection Personal Identification Number (IP PIN). The IRS offers an online account that can show an IP PIN immediately and an online Form 15227 submission for eligible applicants who cannot establish an account. Faxing the PDF is another option.

Check the form before faxing

  • Check all eligibility criteria on the current form, including the income limit, an SSN or ITIN, phone access, and that no IP PIN has already been assigned to you.
  • Complete the taxpayer and contact fields and the declaration. The IRS will call to verify identity before issuing an IP PIN.
  • Include a cover sheet marked “Confidential.” Choose either mail or fax, not both.
